Job summary

Millennium Physician Group is seeking a Speech and Language Pathologist (PRN) to join our Home Health Department in Port Charlotte, Florida. You will assess, plan, and deliver personalized speech and swallowing therapies in patients' homes, collaborating with a multidisciplinary team to ensure optimal outcomes.

The role offers autonomy and a supportive environment with flexible scheduling. Florida licensure and 1 year of relevant experience are preferred.

Qualifications

• Florida SLP licensure required.
• Minimum 1 year of experience in hospital, rehab, or skilled nursing facilities.
• Strong clinical, communication, and interpersonal skills.

Responsibilities

• Assess speech, language, cognitive-communication, and swallowing in home health patients.
• Develop individualized treatment plans targeting speech, language, fluency, voice, and swallowing goals.
• Provide direct therapy services (articulation, language, fluency, voice, swallowing) using evidence-based practices.
• Educate patients and families on strategies to improve communication and safety.
• Collaborate with physicians, OTs, and PTs to create comprehensive care plans.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries.
• Adhere to HIPAA and Medicare regulations.
