Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to provide one-on-one therapy in Sioux City? Waterloo, IA and surrounding senior living settings. You will evaluate, plan, and deliver evidence-based treatment for older adults in private homes or communities.

Work with an interdisciplinary team, document in the EMR, and have the option to incorporate telehealth to extend access and flexibility while maintaining autonomy over your treatment schedule.

Qualifications

• Licensed Speech-Language Pathologist in state(s) of practice or eligible to obtain licensure
• Degree from an accredited Speech-Language Pathology program
• CPR certification
• Current ASHA CCC
• Proficient with EMR documentation and basic computer skills

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in private residences or senior living communities
• Perform evaluations and develop individualized plans of care
• Deliver therapy sessions to support wellness, independence, and functional mobility
• Document effectively in FOX’s EMR system per regulatory standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate
