Job summary

FOX Rehabilitation is growing in Maquoketa, IA and is seeking licensed Speech Language Pathologists to deliver one-on-one care to older adults in private residences and senior living communities. You will design individualized plans, perform evaluations, and provide evidence-based therapy with the flexibility to manage your own schedule.

Telehealth may be incorporated to extend access and continuity of care.

Qualifications

• Active state licensure in IA or eligibility to obtain
• Current CPR certification
• Degree from accredited SLP program
• Familiarity with EMR systems

Responsibilities

• Provide one-on-one SLP evaluations and treatment to older adults in homes or senior living communities
• Develop individualized care plans and deliver evidence-based therapy
• Document progress in FOX EMR per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Incorporate telehealth when appropriate to extend access
