Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to provide in-home, senior living, and virtual care to older adults in Winter Garden, FL. You will assess, diagnose, and treat communication and swallowing disorders across home and community settings.

You will work with an interdisciplinary team, document care in the EMR, and enjoy a flexible schedule with meaningful outcomes for patients and families. Telehealth options enhance access and continuity of care.

Qualifications

• Active Speech Language Pathology license in the practicing state or eligibility.
• Degree from an accredited Speech Language Pathology program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Certificate of Clinical Competence (ASHA CCC)

Responsibilities

• Provide one-on-one speech language pathology services to older adults in homes or senior living settings.
• Perform evaluations and develop individualized care plans.
• Deliver therapy sessions to improve mobility, independence and quality of life.
• Document in FOX EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs and Fitness Specialists.
• Educate patients, caregivers and staff on safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
