Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to deliver in-home SLPT services, evaluate and treat patients, and participate in care planning under the Clinical Manager. You will work with a multidisciplinary team to enhance communication, swallowing and quality of life in patients' homes.

Minimum qualifications include Certification of Clinical Competence in SLP or Audiology, current unrestricted license, CPR, and a valid driver's license with auto insurance and reliable

Responsibilities

• Evaluate and provide speech-language pathology services in the home or facility
• Participate in development and review of Plan of Treatment and Plan of Care
• Assess, treat, and monitor disorders of speech, voice, language, hearing and swallowing
• Administer and interpret diagnostic tests and applications of treatments
• Observe, record and report changes to supervisor/physician
• Provide instruction on alternative communication systems when appropriate
• Counsel patients, families and healthcare staff
• Maintain and submit documentation per company policy
• Prepare written reports of evaluations, visits, and progress
• Participate in care coordination and discharge planning
• Maintain confidentiality and appropriate information sharing
