Job summary

The Legacy Senior Communities in Plano, TX seeks a Speech Pathologist-Therapist/Audiologist to provide in-home speech pathology services under physician orders. You will oversee care plans, supervise staff, and document patient progress in the home setting.

Requirements include CCC from ASHA, Texas license, CPR, and a Master’s in Speech-Language Pathology, with 1 year of home health and supervised experience. Collaboration with a multidisciplinary team is essential.

Qualifications

• CCC certification from ASHA.
• Current license in Texas.
• CPR certification.
• Master's degree in Speech-Language Pathology.
• Bachelor's in Speech and Hearing Therapy.
• At least 1 year of supervised work experience.
• At least 1 year of Home Health experience.

Responsibilities

• Develop and implement the interdisciplinary client care plan as it pertains to Speech Therapy.
• Evaluate effectiveness of Speech Pathology intervention and update the plan as needed.
• Write clinical/progress notes within 24-48 hours and file in client records.
• Coordinate visits with Intake Coordinator and other personnel as needed.
• Discharge planning and completion of discharge summaries with the physician.
• Educate staff on Speech Pathology techniques and participate in team meetings.
