Job summary

Home & Health Care Management, Inc. in Chico, CA is seeking a Master's or Bachelor's prepared Social Worker to join our Care Management Team for comprehensive case management of older adults, children and individuals with disabilities.

This full-time role supports a multidisciplinary team, coordinates services across Medi-Cal, Medicare, and community resources while promoting client well‑being. CPR within 30 days is required.

Qualifications

• Master's or Bachelor’s degree in Social Work or related field.
• CPR certification required within 30 days of hire.
• Valid California Driver’s License and ability to use own vehicle for travel.

Responsibilities

• Assist team by locating community resources (housing, equipment, transportation, basic needs).
• Advocate to ensure timely medical services for participants in the community.
• Respond to questions from CMT, participants, and other entities for coordinated care.
• Educate participants on benefits of services and community supports.
• Assist with applications (Medi-Cal, IHSS, CalFresh, Regional Center) and coordinate transportation.
