Job summary

Enloe Medical Center, Inc. in Chico, CA, is seeking a Social Worker for Case Management on a per diem, part-time basis to coordinate patient care across episodes of illness with the patient, family, physicians and payors.

The role requires a Master’s in Social Work, CPR within 3 months, and experience in acute medical care. Strong psychosocial assessment, crisis intervention, and effective communication are essential. Proficiency with EPIC/MIDAS is expected.

Qualifications

• Master’s degree in Social Work required or completed within 2 years of hire
• Two years experience in medical social work or counseling preferred
• Current CPR recognition within 3 months of hire

Responsibilities

• Coordinate care and services for assigned patients across episodes of illness
• Collaborate with patient, family, physician, care team and payors
• Utilize assessment, planning, intervention and evaluation processes
• Lead and participate in multidisciplinary care planning to optimize outcomes
