Job summary

Home & Health Care Management, Inc. in Sacramento seeks a Masters or Bachelors level Social Worker to join the Care Management Team to provide comprehensive case management for clients in-home.

You will coordinate resources across Medi-Cal, Medicare, and other programs, supporting older adults, children, and individuals with disabilities. This full-time, non-remote role requires working from our Sacramento office, with travel to client homes within Sacramento County.

Qualifications

• Master's or Bachelor's degree in Social Work or related field.
• Detail oriented and computer literate.
• Valid California Driver’s License with clean DMV.
• Own vehicle and proof of current auto insurance.
• CPR certificate within 30 days of hire.

Responsibilities

• Assist the team with locating community resources (housing, equipment, care providers) to meet participant needs.
• Advocate to ensure timely medical services for participants to remain safe in the community.
• Respond to questions from CMT, participants, and others for smooth care coordination.
• Provide education to participants about benefits of services and community supports.
• Assist participants with applications (Medi-Cal, IHSS, Calfresh, Regional Center) and arrange transportation.
• Document all consumer interactions and maintain timely reporting and documentation.
• Observe participants behavior and report significant changes to supervisor or team.
• Daily case conference with CMT about shared participants.
• Perform other related duties as assigned.

Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.

Overview of the Role

We want to hire bachelor’s prepared Social Workers (or another related field such as Gerontology) who will work with a Care Management Team (CMT) consisting of Master’s or Bachelor’s prepared Social Worker and Registered Nurse team to provide comprehensive case management services to target population.

You are detail oriented, computer literate, and want to work as part of a management team supervising the care of clients who are receiving home and community-based services.

You are good at problem solving, knowledgeable about community resources, and enjoy helping clients solve social determinants of health care problems.

You like working 8am-5pm on weekdays with no weekend work.

Responsibilities:

• Assists the team with finding community resources (i.e., housing; medical equipment and supplies; primary care physician and specialists; essential items like food, clothing, and household goods; medical transportation; and caregivers, etc.) needed to meet the participant’s needs.
• Engages in advocacy efforts to ensure participants receive timely medical services to remain safe in the community
• Responds timely to questions and enquiries from CMT, participants, and other entities for smooth patient care coordination
• Provides education to participants so they can better understand the benefits of services and community supports.
• Assists participants with applications (i.e., Medi-Cal; IHSS; Calfresh; and Regional Center, etc.). Explains eligibility requirements, coordinates transportation for participants and other basic needs of support
• Document all consumer interaction and maintain timely reporting and documentation requirements
• Observes participants behavior; reports significant changes to supervisor or team; documents appropriately
• Daily case conference with CMT about shared participants
• Performs other related duties as assigned.

Essential Requirements:

• Master's degree or Bachelor’s degree in Social Work or another related field

• Skilled in operating a personal computer utilizing a variety of standard software

• Valid California Driver’s License and eligible to be insured under our liability policy with a clean DMV report
• Your own vehicle and proof of current auto insurance

• CPR certificate required within 30 days of hire

Location and Service Region:

This is not a remote role, and you are required to work from our Sacramento office.

You will need your own car and be able to travel to client’s homes in Sacramento County and the greater surrounding area. Work is performed in a community and office environment.

PPE is provided to all employees.

Transporting clients with personal vehicle is prohibited

Schedule

Full time, 40 hours per week, from Monday through Friday from 8am to 5pm with an hour for lunch. No weekend work.

Currently 20% of the time is related to direct home visits, 80% of the time is telehealth video visits and or calls and documentation in the office.

Physical Requirements:

Job offer is conditional upon successfully passing a medical exam that tests amongst other things the ability to move, visual acuity, and hearing acuity. Physical requirement for this role specifically includes

• Sits, stands, bends, lifts, walks, and moves intermittently during working hours
• Able to lift to 35 pounds without assistance
• Able to drive intermittently

Compensation:

Pay range is $26.00 - $33.00 / hour. Exact compensation may vary based on skills and experience.
