LiveWell Partners is seeking a Home Health Speech Therapist (SLP) to evaluate patients in the home care setting and develop individualized treatment plans in collaboration with the interdisciplinary care team.

You will implement therapy services, document progress, and coordinate with physicians to support patients’ goals and rehab potential, while ensuring smooth transitions post-discharge and contributing to a positive care environment.

Qualifications

• Minimum of 2 years of speech therapy experience, with a preference for acute or post-acute care.
• Active state licensure in Speech-Language Pathology is required.
• CPR certification and a valid driver’s license; proof of auto insurance coverage.

Responsabilités

• Establish patient goals and help patients meet them by providing quality care.
• Determine speech therapy treatment plans in consultation with the care team and physicians.
• Prepare written plan of treatment for physician signature, including clinical findings and goals.
• Complete discharge planning with the care team and physicians to ensure continuation of therapy after discharge.
• Participate in case conferences.
• Document patient care in charting and department records.
• Perform other duties as reasonably assigned.
