Job summary

Residential Home Health and Residential Hospice is seeking a Home Health Speech Therapist (SLP) PRN to provide skilled care in patients’ homes in Shiawassee County and surrounding areas, Owosso, MI. This role requires driving to patient locations and collaborating with an interdisciplinary team.

The position focuses on evaluations, individualized treatment plans, and caregiver education, with emphasis on safety and quality of life for patients.

Qualifications

• Active Speech Language Pathologist, SLP license in the state you work.
• One year of experience as a Speech Language Pathologist.
• A valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations assessing speech, language, cognitive-communication, and swallowing function, developing individualized treatment plans to support safety, independence, and quality of life
• Educate and empower patients and caregivers on therapeutic exercises
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
