CenterWell Home Health is seeking a Home Health Speech-Language Pathologist to evaluate, treat, and support patients in their homes or facilities. You will participate in treatment plans and coordinate care with the clinical team.

You will assess speech, voice, language, hearing and swallowing disorders, administer diagnostic tests, and maintain detailed documentation while delivering compassionate care to improve patients’ independence and quality of life.

Qualifications

• Certification: ASHA CCC or equivalent.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted licensure; CPR certification.

Responsabilités

• Evaluate and provide speech-language pathology services in home or facility.
• Develop and periodically review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and screenings.
• Observe and report changes in patient condition to supervisor/physician.
• Provide instruction on alternative communication methods when appropriate.
• Maintain and submit documentation including evaluations, visits, and progress reports.
