Job summary

FOX Rehabilitation seeks a licensed Speech Language Pathologist to provide in-home and senior living care across Mentor, Wickliffe, Willoughby and Eastlake, OH. You will evaluate, plan, and deliver one-on-one therapy, focused on improving communication, safety, and quality of life in patients' homes.

Qualifications

• Valid SLP license in the state(s) of practice or eligibility to obtain.
• Current ASHA CCC certification.
• CPR certification current.
• Proficient with EMR systems.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living communities.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy to support wellness, mobility and independence.
• Document in FOX's EMR in compliance with standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and staff to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
