Akron Children's Hospital is seeking a Home Health Skilled Nursing RN to deliver patient-centered care in the home setting. The RN visits multiple patients each day and collaborates with physicians, therapists, caregivers, and community resources to promote optimal patient outcomes while ensuring care is delivered safely and in accordance with agency, state, and federal standards.

This full-time, dayshift position requires Ohio RN licensure, BSN preferred or obtained within five years, and BLS

Qualifications

• BSN preferred or must be obtained within five years of hire.
• Licensed and registered as an Ohio professional nurse.
• BLS certification from the American Heart Association is required.
• Minimum one year of in-hospital or home care nursing; interdisciplinary teamwork.

Responsabilités

• Provide skilled nursing care meeting professional, regulatory, and organizational standards.
• Complete comprehensive patient assessments and ongoing reassessments to guide care planning.
• Develop, implement, evaluate, and revise individualized plans of care with physicians and the interdisciplinary team.
• Coordinate care with physicians, therapists, caregivers, and community resources.
• Use nursing process, clinical judgment to make independent decisions.
• Educate patients, families, and caregivers on disease management and treatments.
• Maintain accurate documentation and communicate status changes with the team.
• Participate in the on-call rotation for patient care coverage.
• Engage in QAPI activities and required staff meetings and training.
• Perform other duties as assigned.
