Job summary

Akron Children's Hospital Home Health is seeking a Skilled Nursing Registered Nurse (RN) to provide comprehensive, patient-centered care in home settings. The RN visits multiple patients daily, collaborates with physicians, therapists, and caregivers, and ensures safe, high-quality care in accordance with state and federal standards.

Requirements include a BSN (preferred) or registration within five years of hire, Ohio RN license, and BLS certification.

Qualifications

• BSN preferred or must be obtained within five years of hire.
• Licensed and currently registered as a professional nurse in Ohio.
• Board-certified BLS by American Heart Association.
• Minimum one year of in-hospital or home care nursing experience; teamwork preferred.

Responsibilities

• Provide skilled nursing care meeting standards and regulations.
• Perform comprehensive patient assessments and ongoing reassessments.
• Develop, implement, and revise individualized care plans with the team.
• Coordinate care with physicians, therapies, and community resources.
• Utilize nursing process and clinical judgment for decisions.
• Educate patients and families on disease management and medications.
• Maintain accurate clinical documentation and communicate changes.
• Participate in on-call rotation and training sessions.
• Support Quality Improvement activities and meetings.
• Perform other duties as assigned.
