Job summary

Akron Children’s Hospital is seeking a Skilled Nursing Registered Nurse to provide comprehensive, patient-centered care in the home setting.

The RN will visit multiple patients daily and collaborate with physicians, therapists, caregivers, and community resources to promote optimal patient outcomes while ensuring care is delivered safely, effectively, and in accordance with agency, state, and federal standards.

Qualifications

• State of Ohio RN license required.
• BLS certification required (American Heart Association).
• Minimum 1 year of in-hospital or home care nursing experience.
• BSN preferred or must be obtained within five years of hire.

Responsibilities

• Provide skilled nursing care meeting standards.
• Complete patient assessments and ongoing reassessments.
• Develop and revise care plans with the interdisciplinary team.
• Carry out physician orders and coordinate with therapy and community providers.
• Use nursing process and sound judgment for independent decisions.
• Educate patients and caregivers on disease management and treatments.
• Maintain accurate clinical documentation and communicate status changes.
• Participate in on-call rotation and QAPI activities.
• Perform other duties as assigned.
