Qualifikationen

• Experience with interdisciplinary teams preferred.

• Provide skilled nursing care meeting standards and care plans.
• Complete assessments and ongoing reassessments to guide care.
• Develop and revise individualized care plans with the team.
• Coordinate physician orders and therapy/community services.
• Use nursing process and clinical judgment for decisions.
• Educate patients and families on disease management and meds.
• Maintain timely, accurate clinical documentation and status updates.
• Participate in on-call rotations for coverage.
• Engage in QAPI activities and required meetings.
• Perform additional duties as assigned.
