Job summary

AHRC NYC is seeking a Field Supervisor, In-Home Services to lead and supervise community-based care for a caseload of 50-60 individuals. The role requires coordinating assessment, plan development, and documentation while guiding direct care staff.

Candidates should have a Bachelor’s degree with supervisory experience in home care or strong direct care background, excellent communication, and willingness to travel. Bilingual skills are a plus.

Qualifications

• Bachelor's Degree with 2-3 years supervisory experience in home care, or 4-5 years direct work plus 1-2 years supervisory.
• Strong computer and statistical reporting skills essential; ability to communicate effectively with diverse stakeholders.

Responsibilities

• Oversee community habilitation, in-home respite, Bridges to Health and TBI services for a caseload of 50-60 individuals; develop and monitor service plans and document accordingly.
• Conduct home visits to ensure proper implementation of plans and staff interactions.
• Provide on-site coaching and training; coordinate staff development with Training Coordinator.
• Participate in weekly staff meetings and support department or programmatic projects as needed.
