FOX Rehabilitation is expanding in Commack, NY, delivering one‑on‑one physical therapy to older adults in their homes or senior living communities. Therapists set their own schedules, treat patients in private residences, and may utilize telehealth to extend access and continuity of care.

The role emphasizes autonomy, meaningful patient outcomes, and strong patient relationships, with opportunities for in‑home and community visits and flexible scheduling.

Qualifications

• Licensed Physical Therapist in NY or eligible to obtain licensure.
• Graduate from an accredited Physical Therapy program.
• Current CPR certification.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence‑based treatment.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate.
