Job summary

Fox Rehabilitation is seeking a licensed Physical Therapist to deliver in-home and senior living PT services in Jefferson, WI. You will perform evaluations, create plans of care, and provide evidence-based treatments to older adults in their homes or communities.

The role emphasizes autonomy, scheduling flexibility, and a strong focus on meaningful patient outcomes, with optional telehealth integration to extend access and continuity of care.

Qualifications

• Licensed physical therapist in Wisconsin or eligible to obtain licensure.
• Degree from an accredited physical therapy program.
• Current CPR certification.

Responsibilities

• Provide one-on-one PT to older adults in private homes or senior living communities.
• Develop and implement individualized plans of care with evidence-based treatments.
• Document in FOX EMR per regulatory standards.
• Collaborate with an interdisciplinary team (PT/OT/SLP) for coordinated care.
• Educate patients and caregivers to promote safety and independence.
