Qualifications

• Licensed physical therapist in Wisconsin.
• Graduate of an accredited physical therapy program.
• Ability to work autonomously in home and senior living settings.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Document therapy sessions in the EMR following regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Offer telehealth/virtual care when appropriate to extend access.
• Build and maintain strong, lasting patient relationships.
