Job summary

Fox Rehabilitation is seeking licensed Physical Therapists to deliver in-home and senior living care across Stoughton, WI. You will conduct evaluations, create care plans, and provide evidence-based therapy to improve mobility and independence for older adults.

There is flexibility to manage your own schedule and incorporate telehealth where appropriate, with a supportive interdisciplinary team and clinician-driven culture.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and independence.
• Document effectively within FOX’s EMR system in accordance with regulations.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
