Job summary

FOX Rehabilitation in Jefferson, WI is expanding its geriatric home-care practice. As a Physical Therapist, you will deliver one-on-one care to older adults in private homes or senior living communities, designing personalized plans to improve mobility and independence.

Work with an interdisciplinary team, utilize telehealth when suitable, and enjoy the flexibility to manage your own schedule while building lasting patient relationships.

Qualifications

• Licensed Physical Therapist with eligibility to obtain license in the practice state(s).
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency; CPR certification current.

Responsibilities

• Provide one-on-one PT to older adults in home or senior living settings.
• Perform evaluations, develop individualized care plans, and deliver evidence-based treatment.
• Deliver therapy to support mobility, independence, and safety; document in EMR.
• Collaborate with interdisciplinary team and educate patients, caregivers, and staff.
• Offer telehealth/virtual care when appropriate to extend access.
