Job summary

TEEMA Solutions Group is seeking a Registered Nurse - Home Care in Mesa, AZ to provide skilled nursing care in patients' homes, perform comprehensive assessments, and coordinate with physicians and other home health disciplines.

The role requires an RN license, BLS certification, and a degree in nursing with 1–2 years of relevant experience. The nurse will administer medications, educate families, and document progress per regulatory standards.

Qualifications

• Associate's or Bachelor's Degree in Nursing is required.

Responsibilities

• Conducts in-home patient assessments and uses OASIS when applicable.
• Develops and updates plans of care with physicians.
• Administers medications and wound care in home settings.
• Educates patients and caregivers on disease management and safety.
• Coordinates care with physical therapy, social work, and other team members.
• Documents visits and progress in compliance with regulations.
