Job summary

Qualifications

• Licensed Registered Nurse in Arizona to practice.
• Experience with home health or in-home care is beneficial.
• Ability to document and communicate patient status effectively.

Responsibilities

• Provide skilled nursing services in patients' homes under physician orders and care plan.
• Observe, assess, diagnose nursing needs and educate patients and families.
• Initiate SOC/ROCs and manage care plans per agency policy.
• Update care plans based on patient responses and physician orders.
• Maintain accurate records and collaborate with care team.
