Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current CPR certification.
• Ability to function in diverse home settings and communicate effectively.

Responsibilities

• Provide high-quality clinical nursing services within scope and infection control standards.
• Coordinate care with the patient/client's care team from admission to discharge.
• Complete nursing assessments per federal/state program requirements and payer needs.
• Ensure patient eligibility and medical necessity for services as defined by payer and agency policy.
• Develop and revise individualized plans of care with other providers.
• Ensure plan of care frequency and duration meets patient needs and obtain physician approval for revisions.
