Job summary

All Advanced Home Care Inc. is seeking a registered nurse to join our growing home health team. The RN will develop and implement individualized care plans, perform in-home visits, and collaborate with a multidisciplinary team to ensure high-quality nursing care for our patients.

Eligible candidates have a Florida RN or LPN license, RN- or ADN-based degree, 1–2 years of nursing experience (home health preferred), and a current BLS certification. A valid driver’s license is required.

Qualifications

• Valid Florida RN or LPN license.
• RN - Bachelor's or Associate's Degree in Nursing.
• 1–2 years of nursing experience; home health preferred.
• Current BLS certification.
• Licensed driver with reliable vehicle.
• Strong observation, communication, problem-solving, math, and nursing skills.
• Ability to work independently with flexible schedules.
• Knowledge of health and safety guidelines and HIPAA privacy rule.

Responsibilities

• Conduct in-home assessments and develop a care plan using OASIS data elements.
• Perform in-home visits and provide skilled nursing care, adjusting plans as needed.
• Deliver compassionate, patient-centered care addressing physical, emotional, and psychosocial needs.
• Educate patients and families on disease prevention and wellness.
• Document observations and treatments in electronic health records per agency protocols.
• Identify discharge planning needs and coordinate prior to discharge.
• Participate in case conferences and team meetings.
• Follow infection control protocols and HIPAA privacy requirements.
