Job summary

Giving Home Health Care in Alcalde, NM is seeking a compassionate Nurse (RN or LPN) to deliver in-home care under a physician care plan. This is a 1099 contract position serving patients in their homes, with flexible scheduling totaling 8 hours per month.

You will collaborate with CNAs, HHAs, Case Managers, and physicians to ensure high-quality care, perform assessments, manage medications, and document care in the patient record. Background checks apply.

Qualifications

• Graduate of an accredited nursing program (ASN or BSN).
• Current RN or LPN license (state or compact).
• CPR certification current.
• TB test and physical exam current.
• Maintain required professional liability insurance.

Responsibilities

• Execute physician prescribed plans of care.
• Document care provided in the system of record.
• Administer medications and therapies as prescribed.
• Conduct patient assessments and coordinate care.
• Assist with activities of daily living.
• Monitor vitals and intake/output; assess skin integrity.
• Communicate with family, physicians, and case managers.
• Provide patient and family education and Home Health Aide training.
• Supervise CNAs and Personal Care Attendants.
