Job summary

Giving Home Health Care is seeking a compassionate RN or LPN to provide in-home patient care in Alcalde, NM, under a physician care plan. This is a 1099 contract role delivering care at the patient’s residence, with 2 hours of care per month on Tuesdays starting at 9:00 or 9:30 AM.

You will collaborate with CNAs, HHAs, case managers, and physicians to ensure quality care. The role requires an active nursing license, CPR, background check clearance, and ability to commute to patient homes.

Qualifications

• Current RN or LPN license in good standing.
• CPR certification.
• Ability to travel to patient homes in Alcalde, NM.
• Strong communication and interpersonal skills.
• Ability to lift up to 50 lbs and assist with daily living activities.

Responsibilities

• Execute physician-prescribed plans of care.
• Document care provided in the patient record.
• Administer medications and treatments as ordered.
• Assess patients and coordinate care with the care team.
• Support patients with activities of daily living.
• Monitor vitals and intake/output.
• Assess skin integrity and wound care.
• Communicate with family and physicians about patient status.
• Educate patients and families on care plans.
• Work one-on-one in the patient’s home with the patient.
