Job summary

American Medical Home Health & Hospice Services seeks an experienced Registered Nurse to join the CLASS program team serving Corpus Christi and nearby communities, providing in-home nursing care to individuals with developmental disabilities or related conditions.

The RNwill conduct assessments, develop ISPs, and deliver skilled nursing services while collaborating with the interdisciplinary team. Strong clinical judgment and familiarity with home health are essential.

Qualifications

Active Texas RN license and current CPR certification.
Minimum 1 year nursing experience in home health, hospice, or community-based settings.
Experience with mediports required.
Strong knowledge of CLASS program requirements and waiver service delivery preferred.

Responsibilities

Conduct initial and ongoing nursing assessments for CLASS participants.
Develop, implement, and update ISPs with the interdisciplinary team.
Provide skilled nursing services including G-tube/J-tube management and mediport care.
Educate participants and families on medication administration and infection control.
Collaborate with physicians, therapists, and direct care staff for coordinated care.
Document assessments, visits, and communications per state regulations and agency policies.
Ensure care aligns with Texas CLASS Program guidelines and standards.
Perform supervisory visits to verify care plan compliance and attenant adherence.
