Job summary

American Medical Home Health Services is seeking a compassionate Registered Nurse (RN) to join our Home Health team serving Corpus Christi, Beeville, Mathis, Sinton, and surrounding communities.

You will provide skilled nursing care in patients' homes, helping maintain independence and improve quality of life. This field-based role requires organization, patience, and the ability to work independently with support from an integrated clinical team.

Qualifications

• Current and valid TX RN license.
• Minimum of 1 year nursing experience preferred.
• Home health experience preferred.
• Knowledge of OASIS and home health regulations preferred.
• Ability to travel across Corpus Christi, Beeville, Mathis, Sinton, and surrounding areas.

Responsibilities

• Complete comprehensive skilled nursing assessments in the patient’s home.
• Develop, update, and follow individualized plans of care.
• Provide skilled nursing visits in accordance with physician orders and home health regulations.
• Administer medications and treatments as ordered.
• Monitor vital signs and assess changes in patient condition.
• Provide wound care, dressing changes, disease process teaching, medication education, and patient/caregiver instruction.
• Perform venipuncture, lab draws, and IV-related services as needed.
• Complete OASIS assessments and required home health documentation accurately and timely.
• Coordinate care with physicians, therapists, aides, office staff, and other members of the interdisciplinary team.
• Communicate patient changes, concerns, and needs promptly to the appropriate team members.
• Promote patient safety, compliance, and quality outcomes.
