Professional Case Management is seeking registered nurses to provide in-home health care across Ohio, including Portsmouth. You will build one-on-one relationships with patients and deliver care in the comfort of their homes while setting your own schedule.

Qualifications

• Graduate of a state-approved school of professional nursing.
• Current, unrestricted RN license in Ohio or practicing state.
• Newly licensed grads encouraged to apply with mentorship offered.
• CPR certification and TB test completed or reimbursed.

Responsibilities

• Provide in-home nursing care to patients in the Ohio region.
• Establish strong, one-on-one relationships with patients and families.
• Maintain patient safety and a respectful, culturally sensitive environment.
• Adhere to agency policies and procedures; coordinate with care team.
