UnitedHealth Group in the United States is seeking a Registered Nurse Case Manager (RNCM) for the Care at Home program to deliver patient-centered in-home care, coordinate plans, and collaborate with an interdisciplinary care team.

The RNCM will assess needs, develop and implement plans of care, educate patients and families, and document progress in the electronic medical record while supporting transitions across care settings.

Qualifikationen

• Active unrestricted RN licensure in state of residence and state of assignment.

Aufgaben

• Assess health status within licensure scope and model of care.
• Establish goals to meet identified health care needs.
• Plan, implement, and evaluate responses to the plan of care.
• Work with interdisciplinary team to address medical, functional, and social barriers to care.
• Collaborate with mental health clinicians to bridge mental and physical health.
• Review daily on-call reports and coordinate with APC/RN as clinically indicated.
• Consult with PCP, specialists, or other health professionals as appropriate.
• Refer patients to community resources as needed.
• Facilitate transitions within and between health care settings with the PCP.
