Job summary

CareAparent is seeking a Registered Nurse Case Manager (RNCM) to perform in-home assessments, help create care plans, and supervise HHA and CNA staff across the local area. The role emphasizes educating patients and families while ensuring high-quality, in-home care to help seniors remain at home.

The RNCM will coordinate interdisciplinary referrals and oversee staff, documenting care using the company EMR system.

Qualifications

• Current Minnesota RN license required.
• Experience with seniors and client assessments.
• Excellent customer relations and interpersonal skills.
• Excellent verbal/written communication skills.
• Ability to travel to client homes.
• On-call availability for rotating weekends and holidays.
• Valid driver's license and insured vehicle.

Responsibilities

• Conduct initial home health care OASIS assessments for Starts of Care, Resumptions of Care, and Recertifications.
• Work within local primary zone; cover other zones as needed.
• Develop and monitor a care plan based on physician orders and patient assessments.
• Coordinate referrals to PT/OT, Aroma Therapy, Speech Therapy, and Falls Prevention.
• Perform skilled nursing tasks in coordination with the Clinical Manager and CareAparent scheduling.
• Oversee LPN visits and ensure the plan of care is followed.
• Provide clinical oversight and leadership to licensed and unlicensed clinicians.
• Prepare visit reports, patient records summaries, and lead care team case conferences.
• Coordinate on-site CNA/PCA/HHA visits and ensure plan adherence.
• Report changes in client condition to physicians and Clinical Manager.
• Document assessments and visits using CareAparent EMR.
