Qualifications

Responsibilities

• Complete intake, annual assessment and admission visits.
• Complete required documentation including client assessment forms and multidisciplinary plan of care.
• Communicate with clients’ PCPs and other healthcare providers as needed.
• Educate caregivers on communication tools and electronic visit verification system.
• Conduct regular in-home visits and caregiver education.
• Assess health and environmental changes; review and update plans of care.
