Job summary

CareAparent seeks an RN Case Manager to perform in-home assessments, re-evaluations, and care plan development for seniors to stay safely at home. You will lead care teams, supervise licensed and unlicensed clinicians, and ensure adherence to the plan of care.

Responsibilities include coordinating interdisciplinary services, documenting visits, and educating patients and families. Travel to local client homes is required, with on-call rotation during weekends and holidays.

Qualifications

• Must hold a Minnesota RN license.
• Experience with in-home assessments and care planning preferred.
• Excellent customer relations and communication skills required.
• Willingness to travel to client homes and work on-call for rotating weekends/holidays.

Responsibilities

• Conduct initial home health care OASIS assessments and starts of care.
• Develop and monitor plans of care based on physician orders and client needs.
• Supervise and coordinate care team, including LPNs, CNAs and HHAs.
• Coordinate referrals to PT/OT/Speech and Falls Prevention.
• Document assessments, visits, and care plans in CareAparent EMR.
