Qualifications

• Must have an RN or BSN certification and possess the original document of certification from a state approved training provider.
• Must provide original documents for CT Driver's License or CT State ID and original Social Security Card.
• Documentation of a physical and PPD taken within the past 12 months or willing to obtain quickly.

Responsibilities

• Direct nursing services including periodic health assessments, teaching, skilled intervention, care coordination, and medication management.
• oversee medical services including nursing, therapy, and home health aides to create a specialized care plan.
• Conduct comprehensive clinical assessments and ensure care plans align with hospital, facility, and physician recommendations.
