Job summary

Arosa invites an experienced RN Care Manager to expand our in-home care and care management services. The role blends clinical leadership with outreach to grow client census and strengthen local partnerships.

You will supervise in-home caregivers, conduct regular visits, mentor staff, educate teams on disease processes, and perform hands-on nursing tasks when needed. Collaboration with the IDT is essential as you coordinate care plans and ensure quality patient outcomes.

Qualifications

• RN licensed in the state where applying.
• Minimum 2 years in geriatrics, home care, home health, or care management.
• Strong communication and teamwork skills; professional interfaces with families and physicians.
• Proficient with PC applications.

Responsibilities

• Oversee all in-home caregivers.
• Perform routine supervisory visits per state regulations.
• Mentor caregivers within their assigned region.
• Educate caregivers and staff on clinical issues.
• Provide in-home nursing care as needed.
• Coordinate with the IDT and manage care services.
