Job summary

FreedomCare is seeking a Field Care Specialist I to join our Missouri team in Springfield. The role involves frequent travel to patient homes, managing a caseload, and providing seamless customer service to patients and caregivers.

You will receive in-house training and have opportunities for career development as you support our CDS program. You will perform 5-7 visits daily with a minimum of 25 visits per week, ensuring high-quality care and compliance across assigned cases.

Qualifications

• 1+ years of field-based experience and a demonstrated ability to manage caseloads.
• Ability to assess patient risks and ensure home safety.
• Strong communication skills, both written and verbal.
• Proven time management, multitasking, and problem-solving abilities.
• Self-starter who travels daily and works independently when needed.

Responsibilities

• Manage an assigned patient’s caseload and travel to home visits.
• Schedule and conduct 5-7 home visits daily; minimum 25 weekly visits.
• Serve as the direct point of contact for member questions or concerns.
• Input data, review cases, build rapport with patients and caregivers.
• Collaborate with Care Coordinators to schedule visits and ensure compliance.
• Complete documentation to meet regulatory requirements; review visit outcomes.
