Job summary

CareSource is seeking a Nurse Care Manager to provide care coordination including in-home assessment, planning, facilitation, advocacy and authorization of plan services to meet the member's health needs while promoting quality cost effective outcomes.

Essential functions include assessing needs, coordinating care across care settings, and collaborating with members, families, and the interdisciplinary team. This role requires RN licensure and may involve travel across assigned territory.

Qualifications

• Associates degree in Nursing from an accredited program required.
• Bachelor's degree in Nursing preferred.
• Two years of RN experience required.
• Clinical geriatrics/managed long-term care experience preferred.
• Bilingual capabilities may be required based on operational needs.

Responsibilities

• Assess and coordinate care across hospital, sub-acute, long-term and home settings.
• Authorize covered services and coordinate care with IDT.
• Perform home visits to assess living situation and needs.
• Develop Patient Centered Service Plan with IDT.
• Document care management per company policy and plan.
• Respond to member requests within designated timeframes.
• Identify and address complex care needs at Intensive Care management meetings.
• Travel and mobility requirements as described in the role.
