Job summary

The U.S. Department of Veterans Affairs seeks an In-Home Community Health Coordinator who is a Registered Nurse (RN) to coordinate in-home patient care services including PSHC, Homemaker/HHA, Home Respite, Home Hospice and Home Infusion as prescribed by licensed providers.

Responsibilities include delivering fundamental care, coordinating across care teams, administering medications, and advocating for Veterans to ensure access to required services within the home setting.

Qualifications

• English language proficiency for direct patient care as required by VA standards.
• Graduate of a nursing program approved by a recognized accrediting body (ACEN/CCNE).
• Current, full, active and unrestricted RN license in the United States.
• Foreign nursing education requires credential evaluation per VA guidance.

Responsibilities

• Deliver fundamental knowledge-based nursing care to assigned clients.
• Assess, plan, implement and evaluate care with age-specific components.
• Coordinate in-home care focusing on patient education, self-management and satisfaction.
• Administer medications and procedures following applicable policies.
• Collaborate with interdisciplinary team to improve care outcomes.
• Advocate for Veterans to ensure necessary in-home services are received.
