Job summary

UNC Health in Chapel Hill, NC, invites experienced Registered Nurses to join the Advanced Care at Home program. This hybrid role combines in-home patient visits with remote nursing support to deliver hospital-level care at home.

You will provide nursing care per standards, educate patients and families, and collaborate with an interdisciplinary team to ensure safe, effective discharge planning and care coordination. 36 hours weekly with a hybrid schedule.

Qualifications

• Licensed to practice as a Registered Nurse in North Carolina.
• BLS for Healthcare Provider certification.
• Two years of nursing experience.

Responsibilities

• Provide in-home clinical nursing care consistent with professional standards.
• Educate patients and family members and assess learning needs.
• Collaborate with interdisciplinary team to evaluate and adjust care plans.
• Perform in-home lab draws/point-of-care testing as required.
• Coordinate care with Command Center and Care Managers.
