Qualificações

• Bachelor's or Associate's Degree in Nursing required.
• Licensed to practice as a Registered Nurse in North Carolina.
• Three (3) years RN experience.
• Clinical knowledge required to deliver evidence-based care.
• Promote patient safety and a positive patient experience.
• Flexible and adaptable to a dynamic work environment and schedule across shifts.
• Team player who can also work independently.
• Engage in ongoing education to care for varied patient populations.

Responsabilidades

• Provide high-quality patient care within Advanced Care at Home framework.
• Monitor, assess, plan, intervene, and apply critical thinking for home patient populations.
• Navigate patients and families across Advanced Care at Home phases.
• Respond to biometric data per protocols and ensure care plans are implemented.
• Conduct home and virtual visits as directed by the care team.
• Collaborate with the care team and supplier partners to support safe, evidence-based care.
• Promote safety culture and participate in process improvements.
• Provide virtual protocol-driven assessments and care coordination as needed.
