Job summary

Powerback Rehabilitation To You in Laconia, NH is seeking a Physical Therapist for full-time in-home outpatient therapy. The role involves delivering PT services to older adults in private homes or ILFs, with opportunities for mentorship, CEUs via Medbridge, and visa/relocation support.

The Physical Therapist will assess needs, design and implement therapy plans, educate patients and caregivers, and collaborate with the rehab team to promote independence and safe mobility in the home setting.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy.
• Must have a valid driver’s license or reliable transportation to and from patients’ residence.

Responsibilities

• Provide outpatient rehabilitation services in in‑home settings for older adults.
• Develop and deliver physical therapy programs; refer to other disciplines as needed.
• Instruct and educate patients and caregivers to promote independence and mobility.
• Supervise PT assistants, temporary licensees, students and support personnel as required by state law.
• Adhere to schedules and maintain clinical and billing documentation.
