Job summary

Select Home Health Services, Inc. seeks a skilled Registered Nurse to provide in-home nursing care in the Bloomington area. This PRN position supports multiple patients daily, performing assessments, implementing care plans, and education for patients and caregivers.

You will coordinate services, supervise LPNs/HHAs as needed, and ensure documentation compliance, while maintaining safety and quality standards in home health care.

Qualifications

• Graduate of an accredited school of professional nursing (required).
• Current Registered Nurse license for Indiana (required).
• Minimum of 1 year nursing experience in hospital, facility, or home health setting; home health preferred.
• Valid driver’s license, reliable auto insurance, and transportation required.
• Current up to date TB test.

Responsibilities

• Perform patient assessments and ongoing evaluations.
• Implement physician-ordered plans of care and complete required documentation (OASIS).
• Provide in-home nursing care for multiple patients per day.
• Educate patients and caregivers on medications and disease processes.
• Coordinate patient services and supervise LPNs/HHAs as needed.
• Communicate changes in patient condition to physicians and care teams.
• Maintain compliance with regulations and agency policies.
