Sage GroupHealthCare Staffing is seeking a Home Health Registered Nurse for a 13-week travel/contract position in Bloomington, IN. We are accepting both local and traveler candidates.

Position Name: Registered Nurse Home Health. Location: Bloomington, Indiana. EmploymentType: Full-time, Travel, Contract, Temporary. Pay Rate: Range $2000 to $2080 weekly gross pay based on a 40-hour week. Mileage reimbursement: $0.49/mile.

Qualifications

• RN with 2 years of experience is required.
• Recent/current Home Health nursing experience (1 year in past 3 years) required.
• OASIS experience required.
• Home Care Home Base (HCHB) charting experience required.
• Must have valid driver’s license and personal auto insurance.

Responsibilities

• Provide and coordinate care for assigned patients in their homes.
• Develop and implement nursing care plans including Start of Care and admissions.
• Support patient recovery and ability to stay at home to enjoy life.
• Educate patients and families to achieve healthcare goals.
• Case manage and provide clinical oversight to interdisciplinary team.
• Provide nursing care within the standards and scope of nursing practice as defined by state regulations and policies.
• Ensure physicians’ orders are legible and transcribed correctly per policy.
• Monitor patient's status and document findings within 24 hours of visits.
• Notify physicians of significant changes in patient status.
• Cover territories in Bedford/Bloomington and surrounding areas.
