Nuclear Care Partners is seeking a Registered Nurse to provide in-home, one-on-one care for patients in White Rock, NM and surrounding areas. You will join a nurse-led team that prioritizes patient dignity and outcomes, delivering skilled care directly in patients' homes.

The role emphasizes individualized care plans, documentation, and collaboration with interdisciplinary teams, with a focus on compliance with state and federal regulations.

Qualifications

• Active RN license in NM
• Current CPR certification
• Valid driver’s license and insured vehicle
• Home health experience preferred but not required
• Must pass pre-employment screenings

Responsibilities

• Provide skilled, in-home nursing care to assigned patients
• Develop and follow individualized care plans
• Accurately document patient care, observations, and updates
• Collaborate with interdisciplinary care teams
• Maintain compliance with DOL, CMS, and state regulatory requirements
