Job summary

Nuclear Care Partners is seeking a Licensed Practical Nurse (LPN) to provide in-home nursing care to patients in White Rock, NM and surrounding areas. You will work with a nurse-led team that values patient dignity, outcomes, and compassionate care in patients' homes.

Requirements include an active NM LPN license, CPR certification, and a valid driver's license with access to a reliable vehicle. Home health experience is a plus, and opportunities for growth and mentorship are available.

Qualifications

• Current LPN license in NM.
• CPR certification is required.
• Reliable, insured vehicle and valid driver's license.

Responsibilities

• Provide skilled, in-home nursing care to assigned patients.
• Develop and follow individualized care plans.

• Collaborate with interdisciplinary care teams.
