Job summary

Pathways Healthcare, a home health and hospice leader, is seeking a Registered Nurse or Licensed Practical Nurse to deliver compassionate in-home care in Westwood, MA and surrounding areas. This full-time position offers a Monday–Friday schedule and competitive compensation.

Salary range listed between $75,000.00 and $100,000.00 with benefits, PTO, and professional development opportunities. Join a patient-centered team focused on dignity and quality care.

Qualifications

• 2+ years' nursing experience, preferably in geriatrics is desirable.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Assesses and evaluates patient’s status by writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals based on nursing diagnosis.
• Initiates preventive and rehabilitative nursing procedures; administers medications as prescribed.
• Completes and submits accurate clinical notes; records pain/symptom changes.
• Communicates with physician regarding patient needs and changes.
• Provides a safe environment; attends interdisciplinary group meetings.
• Participates in on-call duties.
• Arranges equipment and services; supervises ancillary personnel.
