Kahak Health Care Services is seeking an Occupational Therapist to provide skilled OT services in patients' homes in the Washington, DC area. You will evaluate, plan, implement, and document care while coordinating with physicians and the care team.

The role emphasizes maximizing independence, meeting physician orders, and supporting the patient and family in home-based therapy. A valid DC license and CPR certification are required.

Formación

• Must be a graduate of an Occupational Therapy school, approved by the AMA in collaboration with AOTA.
• Preferably a member of the American Occupational Therapy Association.
• Must be licensed or permitted to practice occupational therapy under DC HORA.
• Minimum of one year of institutional/home health experience.
• Criminal background check required.
• Current CPR certification required.

Responsabilidades

• Provides skilled occupational therapy service to patients in their homes in accordance with physician's orders.
• Assists in evaluation of patient's function by applying diagnostic and prognostic procedures.
• Conducts initial evaluation and prepares clinical/progress notes.
• Conducts ongoing comprehensive assessments of the patient’s needs.
• Evaluates home and family situation to determine required instruction and community services.
• Develops and implements interdisciplinary care plan to include Occupational Therapy in consultation with the physician.
• Instructs patient in prescribed therapeutic self-care and activities to improve independence.
• Instructs in use of wheelchairs, braces, splints, and prosthetic devices.
• Supervises family and patient in the OT program and related problems at home.
• Establishes goals and performs re-evaluations.
• Arranges outpatient services when needed.
• Evaluates effectiveness of OT intervention and updates care plan.
